The Department of Water and Power Consumer rebate form must be completed for the installation of a pool pump or motor replacement, Variable-Speed or Two Speed installed with an automatic controller. Please remember to send a photocopy of your DWP bill, along with the ORIGINAL copy of your receipt from the installer and the rebate form.
